We provide health management support for insurance companies -
under our banner or your own.
With the ageing of the Australian population and the increased prevalence of chronic disease, demand on the public health system will continue to increase. Health promotion, preventative-health care and better management of chronic disease is required to address this demand.
We offer disease management, hospital substitution and pregnancy support programs to help insurers manage this demand and to drive customer value.
Disease Management
Based upon similar principles as our health coaching program, disease specific telephonic coaching focuses on lifestyle modification, improved self-management and actions that will positively impact the clinical indicators associated with a disease.
Through a series of structured telephonic coaching sessions we can help a participant to better self manage chronic diseases such as diabetes, obesity, arthritis, asthma or heart disease.
This program is delivered through the combination of our qualified nursing staff and allied health professionals (such as Diabetes educators, Nutritionists).
Our program can help improve a client’s quality of life and provide assistance to their carers over a 12 month period. The program is a confidential, telephone-based program staffed by registered nurses and health consultants trained to give extra support and advice to help better manage their condition.
Services include:
- Regular contact from an experienced Registered Nurse.
- A phone-in service to our Registered Nurses for those unexpected questions.
- A comprehensive book on how to live a healthy life with a chronic condition.
- Links to websites for information and support about specific conditions.
The program utilises the same approach as used in our broader health coaching offering, but with a different focus in regards to goal-setting and behaviour change. Essentially, the health consultant assists a person with a more chronic or complex health issue to make achievable changes that improve their quality of life and improve self-management of their health conditions.
Hospital Substitution
This program is offered to private health insurance companies, hospitals and government funding bodies.
The Health Solutions team help shorten a patient's stay or avoid hospital altogether, by placing support services in their home. With the permission of the treating doctor, the patient can choose to recover at home rather than in hospital. The health consultants tailor a package of care services to assist recovery with 100% of the cost for services delivered covered by the funding provider. The value of this program is in the net savings in cost per hospitalisation.
Our hospital substitution program is run by a team of Registered Nurses who liaise with treating doctors and discharge planners in hospitals. They facilitate the early discharge of the patient by co-ordinating the planning and ongoing delivery of authorised nursing and home care services.
So how does it work?
In simple terms, a patient would be identified as a suitable candidate to be discharged and have nominated services provided in their home. The discharge planner would contact our hospital substitution team, a care plan would be agreed and approved by the treating doctor and we would then arrange to have the agreed services delivered in the patient’s home via our national network of over 250 accredited service providers.
Having provided this service (under our old name - Total Health) for around 12,000 hospital episodes over ten years, our program can demonstrate an impact on reduced length of stay, reduced wait times and avoided re-admissions.
Pregnancy Support
Our Pregnancy Program provides information and educational material appropriate to the stage of pregnancy and age of the child.
Experienced midwives deliver the program which extends from conception to the child’s first birthday and includes:
- A dedicated phone line for contact with experienced midwives.
- Comprehensive books on pregnancy & birth.
- A program of outbound calls by a midwife at key stages to support the educational material.
- Access to a web site with online tools and health content customised for pregnancy and parenting.
- An extension of this program is the Maternity Options package, designed to facilitate the safe, early discharge of mothers from hospital and the provision of care services in the home.
