You are required to bring the following items to your visa medical appointment:

• DIAC forms completed as instructed by DIAC.
• Current and valid passport of the person attending the medical examination.
• Credit card used for the payment and a payment receipt if you booked online.
• Eye glasses or contact lenses (if you wear eye glasses or contact lenses).
• Medical doctor reports (if you have any).

It is not necessary for you to fast prior to your appointment and/or blood test. Please ensure that you keep up a normal intake of food and water/fluids. Also, if possible, please do not go to the toilet immediately before your appointment as you will be required to give a urine sample.
