- Form 160 - Radiological report on chest x-ray for an Australian visa - If you are required to have a chest x-ray you must complete Part A and Part E of this form. If you are pregnant or believe that you may be pregnant, you must tell the radiologist. Children 10 years and younger do not normally require a chest x-ray.
